N2 - Background: We report on a case of malignant lymphoma with enlargement of lacrimal glands and extraocular muscles in both eyes. Case Report: A 78-year-old female presented with bilateral swelling of the eyelid and proptosis. Computed tomography revealed enlargement of the lacrimal glands, four rectus muscles, and the belly of the superior oblique muscle in both eyes and swelling of multiple lymph nodes in the abdomen, chest, and neck. These findings suggested malignant lymphoma, and biopsies were performed of the bone marrow, left subaxillary lymph nodes, and right lateral rectus muscle. Histopathologic examination of biopsy specimens from the bone marrow and subaxillary lymph nodes showed the presence of lymphoma cells. The biopsy specimen obtained from the eye muscle was identified as malignant lymphoma. Conclusion: Malignant lymphoma needs to be considered in the differential diagnosis of an enlarged extraocular muscle.
