Abstract
Radiopharmaceutical therapy (RPT) has become a mainstay of modern oncology but remains limited to solid tumors, including orphan diseases such as neuroendocrine neoplasms or common cancers such as prostate carcinoma. In recent years, RPT has been expanded towards the treatment of hematological neoplasms, in particular by targeting C-X-C motif chemokine receptor 4 (CXCR4). To this end, the most comprehensively evaluated theranostic pair is composed of [68Ga]Ga-PentixaFor (for positron emission tomography, PET) and [177Lu]Lu-/[90Y]Y-PentixaTher (for RPT via β-emission). Beyond its anti-lymphoma and anti-tumor effects, this treatment strategy also causes (desired) bone marrow ablation and thus helps condition patients for hematopoietic stem cell transplantation (HSCT) in tandem with planned high-dose chemotherapeutic regimens. In this chapter, we will summarize how PentixaFor/PentixaTher can (a) improve the delineation of the current status of tumor/lymphoma spread, (b) help identify candidates for CXCR4-targeted radioligand therapy, (c) achieve durable anti-lymphoma efficacy, and (d) prepare patients for HSCT. Such a complex, theranostic strategy requires a multidisciplinary team, including experts in hemato-oncology, gastroenterology, endocrinology, nuclear medicine, radiology, medical physics, and radiochemistry.
