Abstract
The authors report the development of a quantitative method to noninvasively evaluate left ventricular (LV) regional pumping ability by duplex Doppler echocardiography. The LV was approximated by three cylindrical shells of equal height from the apex to the central portion of the LV. The pumping ability of each shell was evaluated by determining a pressure developed (PD), which was a result of inertial force generated within the shell to expel the blood flow. From the Doppler measurements of blood flow velocities in the LV, the inertial force was determined by calculating the change in momentum produced by the contracting shell based on the equation of continuity and that of motion. The PD was obtained as the inertial force corrected for the instantaneous cross sectional area of the LV. The PD in each cylindrical shell was calculated in five normal subjects, three patients with old myocardial infarction and a patient with dilated cardiomyopathy.
